Community Support Case Manager

Nature and Scope: This is a professional, administrative, supervisory and support services
position that addresses Community Engagement and Community Services. Work is performed
under the direct supervision of the Community Support Director or their designee and the
general supervision of the Executive Director or their designee. The Case Manager provides
high-level support by conducting research, preparing statistical reports, handling information
request, and performing all case management functions. The Case Manager determines
certification of eligibility, assessment of client needs, formulation of service plans, follow-up
with clients, client counseling, and mobilization of resources in a timely and
consistent manner.
Responsibilities:
1. Accept and record referral, complete initial assessment, is done to determine client
eligibility through a specified and written certification process. Develop profiles and
assess the needs of each client. Identify needs and/or activities or services plans to
address those needs.
2. Identify and mobilize resources. Develop a resource file that can be used to supplement
the services to provide involving other service providers when possible or feasible.
3. Establish and maintain rapport with other Human Service Agencies and apartment
managers while in the process of developing from these sources, direct services for the
client or their families.
4. Develop, maintain, and keep current all confidential files including records, reports, and
case management plans. Include all supporting documentation of client’s status
changes or changes in the services provided when needed or required. Provide follow-
up activity for each family or individuals assisted.
Qualifications:
Bachelor’s degree in Human Services, Psychology, Sociology or related field, with three (3) to
five (5) years of related work experience and/or training.
Must have experience in customer service, quality service and the ability to address
issues and concerns and conflict resolution.
Must be able to travel locally and out of state for meetings as needed or required.
Salary Range: $35,360-40,000 (Annual Salary Range)
